Award Applications

  • Child Passenger Safety Team Award
    The CPS Team Award should be a team/program/agency that shows dedication to the continuation of both local and state CPS programs. Team members should be current in all areas of CPS documentation and activities. The nominee should have developed activities that promote CPS such as CPS check programs, booster seat programs, maintaining participation by current technicians and encouraging recruitment of new technicians. Through their continued work in the community this team/program/agency stands out as a model to other agencies.
  • Child Passenger Safety Technician Award
    The CPS Technician Award will be given to an individual technician who has shown outstanding dedication to the field of Child Passenger Safety by promoting the role of education in CPS through check events, workshops, and other methods. The recipient can be a volunteer or paid staff.
  • Child Passenger Safety Technician Instructor Award
    The CPS Technician Instructor award will be given to a CPSTI who has shown outstanding dedication to the field of Child Passenger Safety by promoting the role of education in CPS through training, check events, workshops and other methods. The recipient of this award should go the extra distance to ensure that the instructor role extends beyond the classroom by working with technicians after certification to enhance their skills at check events and continuing education workshops. The recipient can be a volunteer or paid staff.
  • Kuska Lifetime Achievement Award
    The Kuska Lifetime Achievement Award is the highest individual Child Passenger Safety Award in Illinois and is named in honor of Thelma Kuska, who has donated thousands of volunteer hours to child passenger safety in Illinois. The Kuska Lifetime Achievement Awards nominee should be an individual who portrays extraordinary dedication to the issues of child passenger safety.
  • The Booster Award
    This award is for an individual, business or agency, who is not a CPS Technician or Instructor but has made a significant contribution to Child Passenger Safety in their community.
